There are more than 100 types of arthritic conditions , but osteoarthritis is the most chronic joint degenerative condition. It is typically a non -inflammatory degenerative disease of the joint that deteriorates the articular cartilage, leads to osteophyte formation ( bony projections on the articulating surface) and deformity of joints. 

Osteoarthritis is no longer a medical condition for the elderly (60 or >60), it is now being frequently reported in younger age groups too. Is this an evolutionary drift, or the trends are pointing out to something?
In India, growing obesity rates, vitamin D deficiency, jobs that involve kneeling, squatting or lifting heavy loads, and untreated joint injuries are major factors that can lead to osteoarthritis, affecting almost 15 million people each year. We Indians are more vulnerable to osteoarthritis than people from other parts of the world because of a genetic predisposition toward weaker bone and muscle strength.

  • Improving functional movement
  • Improving muscle strength & endurance

  • Aerobic exercise is important in weight reduction to off-load affected joints and to improve cardiovascular and respiratory health;
  • Flexibility exercises help maintain range of motion and reduce stiffness;
  • Strengthening exercises will provide essential muscular support joints to reduce mal-tracking, absorb impact and maintain function.
TO SQUAT OR NOT TO SQUAT IN OA?
Osteoarthritic patients should strictly avoid weight bearing exercises, as it will further accelerate the condition making it worse. However, biomechanical references support squatting upto 20 degrees. Though it is a controversial intervention, however, basic knowledge of gait (walking pattern) clearly indicates that maximum part of the gait cycle (walk) involves eccentric action of quadriceps ie; the primary muscle that supports the knee joint by unloading it is mostly lengthened during the gait. Thus , training of quadriceps which is actually an integral  part of OA knee rehabilitation will happen only if it is trained in the right pattern and manner.

*Physical therapist must always correlate the severity of the OA & exercise interventions given and accordingly decide the red flags.

Aquatic exercises adds cherry to pie for OA patients. Water is such an underestimated tool of rehabilitation. Aquatic jog and aqua cycling is a wonderful way of relaxing yourself and making the session fruitful for your joints. So if you have a pool nearby, consult your therapist and tailor out your own exercise session!
